Autism spectrum disorder, as its name implies, is an umbrella term for children with different severity and also distinct symptom characteristics. What are key trends in the evaluation and treatment of children with autistic spectrum disorder? That's a very good question, because I think what we've learned in terms of the assessment process is that a multidisciplinary assessment is essential for children who are thought to be different. We get many families - in fact, I'll be seeing someone this afternoon - who feel that their child is different or underperforming in school, or behaving in a rather odd way, and they want to know whether their child is autistic or not. It's every parent's nightmare to have an autistic child because the child is very often unable to have any kind of social relationship with even the parents of the child. And so we do a sort of structured assessment, which has been evolved over many years, and it is a robust methodology, because it gives consistent results, We very rarely... We don't like to give children a diagnostic label like autism, unless we are absolutely certain that they fulfill criteria on full testing, because it's a lifetime diagnosis, it's not just a diagnosis that gets better after six months, It's a lifetime diagnosis, which has enormous impact on schooling, education and future lifestyle. So we have a special social and communication disorder clinic, and the process [of assessment] is important, because it captures the multidisciplinary aspect of it. First of all, the child has a clinical evaluation by one of our senior neuro-developmental pediatricians or psychiatrists, myself, for example, or Professor David Skuse. Secondly, they do ADOS, which is Autism Diagnostic Observation Schedule, which is a structured series of activities, which we video, so we can review the child's response to the various different questions and activities. Then there is a 3-D interview, which is a detailed computer-based interview, which has a lot of algorithms built into the program, which gives the child a score in various key areas. And then finally there's a psychometric profile, which is a cognitive skills profile. So all of those, and a school visit, if there is any doubt about the diagnosis. We would like to see children at school. So all of those processes are then brought together into a point, where one can actually offer a firm diagnosis based on the evidence that we've established. Or not. We are quite good at identifying children who are autistic, because we've been seeing autistic children for such a long time. So it's rare, once the child gets accepted for referral to the clinic, it's quite unusual for us to overturn previous diagnosis, for example.
